New innovative models of care for primary health care: telenursing

Abstract
To comply with the health objectives, the care model must be modified, adapting to current times; It will be necessary to develop information technologies in health to pay attention with added value. Information and Communication Technologies have to play an increasingly important role in the health system in order to improve the health of people in an efficient way, and will represent a paradigm shift for both the sick and the professionals and Health organizations
Objective: to know the best available evidence related to the implementation of the teleengineering / telemedicine program for reducing the burden of chronic diseases, monitoring care and giving greater accessibility to the system. Material and Method: Literature review of the literature. Results: Tele-nursing turned out to be effective, in education and counseling for self-care by nurses who provide education to patients with diabetes, high blood pressure, dyslipidemia, heart failure, decreasing hospital admissions. Reduce the number of visits to the consultation, costs and distances. Discussion: The rapid information on the status of patients is one of their best weapons, it represents an important advance since the current tendency is for patients to improve and take responsibility for their “self-care”.
